2025/2026 Confirmation Program Information

Welcome to the Confirmation Program for the Catholic Parishes of Adams & Cheshire! The program includes the 9th grade students from St. John Paul II Parish in Adams and St. Mary of the Assumption Parish in Cheshire.

If you would like your child(ren) to participate in this year's Confirmation program, please fill out the following form. The fee for the program is $50/student. Some of the information requested on the form appears repetitive, but is necessary for our Sacramental Record keeping.

Please note, your child(ren) will be required to select a sponsor. Each sponsor will be required to obtain proof of standing within his/her own parish and provide their Sacrament Record information including proof of Baptism and Confirmation.
