Faith Formation Registration - 2025/2026 School Year

2025-2026 Program Information

Welcome to Faith Formation at the Catholic Parishes of Adams & Cheshire! All students from both St. Mary's and St. John Paul II Parishes will meet this year for in-person classes on Sundays at St. Mary's in the parish hall.

Grades K-9 will meet on Sunday mornings from 8:30 - 9:30 a.m.

Please register students who will be receiving their First Holy Communion (Grade 2) or Confirmation (Grade 9) on the separate forms available on our website. The information required for these levels is different and an additional fee applies to cover the sacramental program expenses.

If you intend to have your child(ren) participate in Faith Formation this year, please complete this form. Enter the information for your child(ren) along with your parent/guardian information then proceed to the bottom of the form for payment. The fee is $30/one child, $55/two children, $75/three or more children.
